The Shoulder Joint

A ball-and socket joint, it is the most flexible joint in the body. The ball, or head, of the humerus (arm bone) is held against the small, shallow socket (glenoid) by muscles and other soft tissue structures (ligaments or capsule).

A healthy shoulder enables you to move your arm up and down, to the side, across the body, and behind the back.

A healthy shoulder glides smoothly in the socket because the ends of the bones are cushioned by a smooth covering (cartilage), and tissue (synovium) lines the joint.

The muscles and tendons of the rotator cuff hold the head of the arm bone firmly in the socket providing strength, mobility and stability.

Rotator Cuff

·      The tendons of the supraspinatus, infraspinatus, teres minor and subscapularis muscles form the rotator cuff.

·      The most important part is formed by the supraspinatus muscle

·      The rotator cuff plays an important role in stabilising internal and external rotation of approximately 150°, as well as horizontal rotation

What is a Rotator Cuff Tear?

A rupture of the rotator cuff is most commonly a tear of the supraspinatus tendon

90% of all rotator cuff problems are caused by compression of the acromion onto the supraspinatus tendon

The ailment is graded into 3 stages:

o        Stage I: Inflammation

o        Stage II: Fibrosis

o        Stage III: Tear

Stage III is graded into 4 categories depending on the size of the tear

How are Rotator Cuff Ruptures Treated?

A subacromial release is necessary

Small tears are simply sutured

Bigger tears are sutured through the bone by means of anchors (e.g. Zimmer Statak® Soft Tissue Attachment Device or bone tunnels)

Big or recurring tears are sometimes treated with part of the latissimus dorsi muscle

Large tears can also be reinforced by suturing with a shoulder patch, such as Zimmer Collagen Repair Patch

Indications

This product is indicated for reinforcement of the soft tissues which are repaired by suture or suture anchors limited to the supraspinatus during rotator cuff repair surgery.
